Lankenau Medical Center Receives Level 3 Patient Care Medical Home Designation from the National Committee for Quality Assurance

Lankenau Clinical Care Center & Lankenau Medical Associates received Level 3 Patient-Centered Medical Home (PCMH) designation from the National Committee for Quality Assurance (NCQA).  This is the first PCMH designation for Lankenau Medical Center and its integrated health practice.

To receive this designation, Lankenau Clinical Care Center and Lankenau Medical Associates demonstrated that the practice functions as a medical home by using systematic, patient-centered and coordinated care management processes.

      “We are so pleased to have this national recognition for the significant work we have done to transform our practice,” said Madelaine Saldivar, MD, Medical Director of Lankenau Clinical Care Center and Lankenau Medical Associates. “Our patients will benefit greatly from a multidisciplinary approach to their health.”

      With a PCMH care model, the team of health care providers partner with patients and their families to address health concerns while emphasizing social well-being. The team maintains open communications among the hospitals, ambulatory service, the practice and at home, to provide support and improve patients' health.

      “The recognition highlights our emphasis on population health and engaging our patients, their families, and community in promoting their health,” said Chinwe Onyekere, director of Ambulatory Services and Business Development. “During the transformation of our practice, we have developed innovative partnerships with organizations like Philadelphia College of Osteopathic Medicine, the Food Trust, Philadelphia Department of Health and our patients’ insurers to build a comprehensive and sustainable medical home.”
